Step 1: Identification

CAPA Sources: Where Quality Problems Come From
A mature CAPA system draws input from multiple sources across the QMS. If your CAPAs only come from customer complaints, your system is incomplete.
Primary CAPA Input Sources
Customer Complaints: The most visible source. Complaint data should be analyzed both individually (does this single complaint warrant a CAPA?) and in aggregate (do complaint trends reveal a systemic problem?). Under ISO 13485:2016 clause 8.2.2, the organization must handle complaints in accordance with applicable regulatory requirements. The QMSR and EU MDR both require complaint data to feed into CAPA.
Internal and External Audits: Audit findings — whether from internal quality audits (ISO 13485 clause 8.2.4), supplier audits, notified body audits, or regulatory inspections — are a direct CAPA source. Process Monitoring and Trending Data: Statistical process control (SPC) data, yield trends, cycle time trends, environmental monitoring data, and any other quantitative process data. A process showing a statistically significant shift or trend — even if individual results are still within specification — is a candidate for preventive action.
Post-Market Surveillance Data: For the EU MDR, this is mandatory. Post-market surveillance plans, periodic safety update reports (PSURs), post-market clinical follow-up (PMCF) results, and vigilance data must all be evaluated for CAPA input. Literature reviews and analysis of similar devices on the market also feed into this.
Management Review Outputs: Management review (ISO 13485 clause 5.6) reviews aggregated quality data. Decisions and actions from management review can generate CAPAs — for example, if management review identifies that CAPA cycle times have been increasing and determines this is a systemic resource or process problem.
Returned Product Analysis: Products returned from the field — whether due to complaints, warranty claims, or other reasons — should be evaluated. The physical examination of returned product often reveals failure modes that are not apparent from complaint descriptions alone.
Supplier Quality Issues: Recurring supplier nonconformances, supplier audit findings, or changes in supplier performance metrics may warrant a CAPA directed at either the supplier or your incoming inspection/supplier management process.
Risk Management Updates: Changes to the risk management file (ISO 14971) — such as identification of new hazards, changes in severity or probability estimates, or new information from post-market data — can trigger preventive actions.
CAPA Effectiveness Checks
Effectiveness verification is the single most important quality gate in the CAPA process, and it is the step most often done poorly or skipped entirely. The FDA has repeatedly cited manufacturers for failing to verify the effectiveness of corrective and preventive actions.

For Organizations Building a CAPA System from Scratch
Start with the procedure. Write a clear, concise CAPA SOP that defines the trigger criteria, roles and responsibilities, process steps, timelines, forms, and approval requirements. Do not over-engineer it. A 5-page SOP that people actually follow is better than a 30-page SOP that nobody reads.
Create a CAPA form or template. Whether paper-based or electronic, the form should walk the user through each step: problem description, evaluation, investigation, root cause, planned actions, implementation evidence, effectiveness check, and closure.
